For example - please answer the following questions for each of your top 5 issues
IIssue Number (1)
Question (1) with it's associated drop down questions (R,Y,B)
Question (2) with it's (different from above) drop down questions (X,Y,Z)
Question (3) with it's (different from the two above) drop down questions (A,B,C)
What I would like it to do (maybe via logic) is to separate this in the survey such that the respondent would only see the current issue and the survey would be efficient in that I would not have to repeat the same listing of question for each of the top 5 issues - any guidance?
Can this Survey tool create the following "hybrid" response question?

I would suggest using 3D Matrix type of questions in this case. Here is an example of how it could look http://www.keysurvey.com/survey/38810...
To create such layout in a 3D matrix question, you need to customize its scale. Please do the following:
1. Go to the survey Questions page and add a question of 3D Matrix type.
2. Specify question text under step 2. In my example it is "Issue Number (1)" wording.
3. Under step 3 click "Cusotmize"
4. Then click "New Scale" at the top right.
5. On the next page select number of columns under step 1, specify header text for each column under step 2 and click "Format Column" under step 3.
6. On the next page, select "Format each column separately" option under step 1.
7. Under step 2 select column and under step 3 select forma for the column.
8. After you finish customiing column format, please click Save and Back on each page to return to the Edit page
9. Under step 4 specify answer options wording. That would be Question (1), Question (2), etc.
10. Hit "Save."
