anyone know how to add an open text comment field to a rating question?
Please rate .... then comment on reason for rating....is this possible?

It's possible to accomplish through the other question type - 3D Matrix.
You will want to change the question type and customize a new scale:
- click edit next to the needed rate question
- change the question type under the step 1
- under the step 3, click the Customize button
- find the New Scale button to the right and click it
- select the needed number of columns in the scale (and +1 for the comments)
- add the scale titles on step 2
- click the Format Column button at step 3
You will need to select "Format each column separately" on step 1. Select "Pick one" for each column, except the last one. For it, you need the General format, which allows you to leave comments.
Save your scale and question.
